I, the undersigned parent or legal guardian of the above-named minor (hereinafter referred to as "Player"), on behalf of myself, the Player, and our heirs, assigns, and legal representatives, hereby agree as follows:

I hereby authorize each of the coaches, team parents, and any other responsible adults associated with the Lions FC Soccer Academy to act as my agents in their capacity as activity supervisors and vehicle drivers. I further authorize these individuals, as well as the designated Emergency Contact, to consent to any necessary medical, surgical, or dental examination and/or treatment for the Player.

I acknowledge that participation in soccer inherently involves certain risks, including but not limited to travel, play in adverse field conditions, and physical contact. These activities carry the risk of severe, permanent injury, including but not limited to bruises, scrapes, strained or torn muscles, tendons, or ligaments, broken bones, dislocations, joint injuries, concussions, brain injuries, nerve and spinal cord damage, paralysis, and even death. By signing below, I willingly and voluntarily accept and assume all such risks on behalf of the Player.

Make-Up Sessions: A make-up session will only be offered if the coach is unable to attend a scheduled practice. Please note that there are no refunds for sessions canceled due to bad weather or holidays. However, if a session is canceled for either of these reasons, players are welcome to join an afternoon practice session at Lions FC, held at Peacock Park in Coconut Grove, at no extra cost. These sessions take place Monday through Thursday at 4 PM.

Payment Policy: A late fee of $25.00 will be applied if payment is not received within the first 5 days of each month. 

Uniform Requirements: Every player, without exception, must wear shin guards and the official Lions FC practice uniform during all practices.

Payment: I acknowledge and commit to paying the monthly tuition during the active season, even if my child is absent due to school activities, vacations, or other reasons. 

I have carefully read and fully understand the terms and conditions outlined in this Release and Authorization, including the Disclaimer, Assumption of Risk and Waiver, and Policies. I acknowledge that by signing this document, I am voluntarily giving up substantial rights on behalf of myself and the Player. I sign this document freely and without any inducement.
