In accordance with Texas Senate Bill 12 (SB12), a parent or guardian must provide written permission before a student may participate in any club.

This form is required for all K-12 student clubs in Prosper ISD, including curricular, co-curricular, extra-curricular and competitive clubs.

Student emails are not accepted. 

Note:  This form provides parental permission only. 

  • If a club has prerequisites for membership (such as grade eligibility, applications, courses, etc.), those requirements - as listed in the club descriptions below - must be met before the student can be accepted as a member.
  • Completing this form does not enroll a student in the club. Students must still contact the sponsor or attend meetings to join.
