On this form, you can provide us with some information about your family. This is the first step for enrolling in a study at The Music Lab. Once you provide this information, we'll get in touch with you soon to thank you for signing up, give you information about our research, and let you know when appointments are available.

We run our studies every day of the week, including on many holidays and in the evenings — we want to make it as convenient as possible for you to visit our lab!

Some of our studies are only available to children of a particular age. If your child is the right age for one of our studies, we will be able to schedule an appointment right away. If not, don't worry! We'll let you know when your child will next be eligible.

The information you provide on this form is stored securely on our servers and is never shared with anyone. It will be used strictly for the purposes of contacting you about our research. You can decline to participate at any time. If you would like to be removed from our database, no worries! Just let us know.
