Please select and submit the days missed or to be missed for AFTER-SCHOOL rehearsals and performances. For planned absences with doctor's notes, fill out this form, then provide documentation via email or in person to the directors BEFORE your absence. In the case of emergencies or unplanned absences, provide documentation when returning to rehearsals.  Excused absences will be determined at the directors' discretion.
