This form is for professionals to refer someone they work with (age 18+) to the Finding Rhythms: Studio (formerly known as Making Waves) course.

If you wish to refer yourself for the course, please complete the self-referral form instead.

Special category data (e.g. mental health) is optional to provide. If it is not relevant to the individual's participation in the course, we do not require this information to be shared.
