We've updated the form based on member feedback. You'll now be prompted through each section with yes/no replies for each requirement--feel free to log everything at once or come back and do it as you complete items.
We're asking for each family's door code--this is new!--so that it can pull into your dashboard on wpns.org/members where you can check your stats as you wish. (Note that there is a delay from when you log until it shows up on dashboard.)
