Heralders Parent Agreement
I understand that my child's time with the Heralders Children's Choir is a professional music lesson. I will respect this by dropping off and picking up my child on time. Siblings not participating in the choir MUST BE supervised in and outside of the building at all times. *
I understand I am enrolling my children for an entire semester: Fall and/or Spring. Registration is semester based and due at the beginning of each semester. Tuition is semester based but can be paid monthly. Tuition is required even if rehearsals are missed. Missed choir time cannot be made up. *
Tuition is semester based but can be paid monthly. Semester tuition must be paid by the first rehearsal. If paying monthly, tuition is due the first of each month. I understand a late fee of $20 will be assessed if my tuition is later than the 10th of each month. A fee of $30 will be charged for returned checks. *
I release the Heralders, Deborah Smith, and any rehearsal or performance venue and owners from any and all liability or responsibility for injuries occurring to my child before, during or after rehearsal in or around the buildings. In case of accident or illness, I give my permission for those supervising to administer emergency treatment to my child. *
I agree to allow my child's image to be used on the Heralders' website at the discretion of the Director. No names will be used. *
