Book Voucher Request
Please complete this form to request a book voucher.  To receive a book voucher a student must have financial aid that totals more than their costs.  Once the voucher is used, the cost of books will be applied to your ledger and subtracted from any refund.  With the completion of this form, you signify that you understand any and all costs for books applied to your ledger are ultimately your responsibility in the case aid must be reduced or is not received.

To be certain you are eligible for a voucher, please answer these questions:
I have a scholarship or grant which will pay the college (not the student) for books (examples are Careerlink, Workforce Development...)

Your "ID" number.  This is the number that begins with a A and has a series of zeros.  It can be found  by logging into your CAMS portal.  Go to “My Grades”, “My Final Grades”, and look for “Student ID”.  If this number is entered incorrectly, your voucher will not work. *
