80th National Pearl Harbor Remembrance Day Wreath Donor Form
Pacific Historic Parks (PHP), Pearl Harbor National Memorial (NPS), and Navy Region Hawaii do not coordinate wreath orders, but we recognize that the public, civic organizations, and many others like to acknowledge and honor the service and sacrifice of the men and women who were on Oahu that fateful day, and who may have served in various aspects of World War II and the war effort.
With that in mind, our collaborative team has provided some additional information for those interested in donating a wreath for Pearl Harbor Commemorative events. We ask that donors provide us with this supplemental information so that we can assist you in coordinating wreath expectations and deliveries.
While the main ceremony location is still being determined. We will ensure that any donations are routed to the proper place.
Please place your wreath order first and then fill this form out afterwards. We are not collecting any payment information.
Wreath donation coordination is supported with the generous staffing and dedication of Pacific Historic Parks employees.
We will not be able to coordinate any additional wreaths after the deadline or until the number of wreaths reaches a number that pushes us beyond our ability to display. The deadline for Wreath Donors is 5 pm HST Tuesday November 23, 2021.

I understand that sending a wreath to any of the jointly hosted National Pearl Harbor Remembrance Day Ceremonies does not grant myself or my organization reserved seats for any of the ceremonies nor does it guarantee access to any of the events.
I understand that PHP, the National Park Service and the US Navy do not have a preferred vendor, nor are federal agencies allowed to provide recommendations. You are welcome to use your preferred search engine to seek out florists who will deliver to the 96818 area code.
I understand that wreaths that are not disclosed in advance via this form run the risk of being rejected by Pearl Harbor National Memorial and/or Joint Base Pearl Harbor-Hickam Security Staff.
I understand that my wreath must come with and safely stand on a 4 foot (4') wreath stand. Stands higher than 4' will block the historic viewshed of the Park and be more likely to blow over causing damage to your wreath.
Wreath Delivery Instructions
I understand that my wreath must arrive at the designated event location by 2 pm on Sunday December 5 (for the USS Utah Ceremony) or 2 pm Monday December 6 for events taking place on December 7. Wreaths will stay up for 2-4 days based on weather and natural decomposition. Wreaths will be recycled whenever possible.
Preferred Wreath Location
Wreaths will be displayed on the Pearl Harbor Visitor Center Tree of Life Statue and Shoreline, USS Nevada - Hospital Point site, USS Oklahoma Memorial, or the USS Utah Memorial. A select few may be chosen to be placed on the USS Arizona Memorial by event support staff. This decision is at the discretion of the event leadership team and does not entitle the donor to any special or unique access. We can not guarantee a specific location or placement.
