Use this form to request meals from the Central Kitchen for your school site.

This form is intended for schools without an on-site production kitchen that receive meals delivered by Community Roots.

Ordering Deadline

To ensure adequate time for menu planning, food purchasing, production scheduling, and delivery coordination, all meal orders must be submitted by Wednesday of the week prior to the week of service.

Submitting this form does not automatically approve your order. Community Roots School Food Services Agency will review your request and contact you if additional information is needed.
