Music Class Scheduling Adjustment Form
This form is to be submit for changes to the class schedule. All requests have to be approved by the Area Head and the Director of the School of Music. Only one request per submissions. Please complete a separate request for each requested change.

Class Notes (This will be printed on the class schedule)
A copy of your responses will be emailed to the address you provided.
