This is a once per year, annual registration form -- for everything. If you are an eligible Colorado veteran or family member please complete this form.
One form for all programs and services. Healing arts, studio or computer access, classes and workshops, transportation, emergency funding and assistance and specialty tools and art supply requests!
Admittance to the studios or classes will not be permitted without annual registration.
- Use this form to register for access to the veterans art center for open studio, classes, computers, etc.
- Use this form to apply for alternative healing arts programs
(i.e. Bodytalk, TaiChi, Acupuncture)
- Use this form to sign up for transportation requests, including rides or bus passes.
- Use this form if you would like to request specialty tools or art supplies. You will be contacted by email for further information if required. A DD214 may be required.
- Use this form if you would like to apply for emergency funding assistance, food or gas cards, or other emergent needs. You will be provided with an additional form for further information and details. A DD214 is mandatory.
This form will only need to be completed once per fiscal year. (7/1-6/30)
