⚠️ GUIDELINES FOR REGULAR VOLUNTEERS:

✅ A DEFINITIVE LIST of volunteers will be published at the main entrance.
❌ NO-ONE will be allowed to volunteer without filling-up this Opt-in Sign-up Voluntary Form.
❌ NO-ONE will be allowed to volunteer outside the published list to maintain proper social distancing.
❌ NO-ONE will be allowed inside the premises with any symptom or recent history of a medical condition, fever, cough, runny nose or sickness; Absolutely NO EXCEPTIONS!

⚠️ GUIDELINES FOR STUDENT VOLUNTEERS:

✅ Students, who are enrolled in e-Learning (Learning from home), are allowed to volunteer with at least ONE accompanying parent.
✅ Students Service Learning (SSL) hours will be awarded to the students enrolled in e-Learning. (Ages 14 & above only). Please bring your own school approved SSL hours forms.
❌ TEMPORARILY, we are restricting any student from volunteering who are living in university campuses or are going to schools in person as many cases are coming out from educational institutions. We will review this restriction as soon as there is more clarity on this matter.

⚠️ GUIDELINES FOR ALL VOLUNTEERS:

🔴 Please go-through the checklist of self-quarantine questions below. If you meet any of the criterion, then please DO NOT volunteer or visit Gurdwara sahib until you are fully cured of the symptoms. NO EXCEPTIONS.
✅ ENTER Gurdwara sahib building through main front entrance only. NO EXCEPTIONS.
✅ EVERY volunteer must wait at the Main Front Entrance for a mandatory touch-less temperature check before proceeding to perform your regular voluntary duties.
✅ MUST bring your own head covers/scarfs.
✅ MASKS, GLOVES are Compulsory. NO EXCEPTIONS.
✅ SANITIZER DISPENSERS have been made available at multiple locations in the Gurdwara sahib premises.
✅ ALL VOLUNTEERS must be at AT LEAST 14 years of age & above. NO EXCEPTIONS.
✅ MUST Keep a safe distance of SIX FEET from each other during all times;
✅ REMOVE your footwear near the main front entrance only;
❌ DO NOT wear anybody's footwear under NO CIRCUMSTANCES.
✅ Frequently WASH hands with soap and water;
✅ PRACTICE STRICT SOCIAL DISTANCING AT ALL TIMES;
✅ STAY SAFE 🙏😇

❌ If you answer YES to any of the following questions, please see a medical professional and REFRAIN from volunteering. 🙏

🟪 Have you or anyone in your household had any of the following symptoms in the last 21 days: sore throat, cough, chills, body aches for unknown reasons, shortness of breath for unknown reasons, loss of smell, loss of taste, runny nose, fever at or greater than 100 degrees Fahrenheit?
🟧 Have you or anyone in your household been tested for COVID-19?
🟥 Have you or anyone in your household visited or received treatment in a hospital, nursing home, long-term care, or other health care facility in the past 30 days?
🟦 Have you or anyone in your household traveled on a cruise ship in the last 21 days?
🟧 Are you or anyone in your household a health care provider or emergency responder?
🟪 Have you or anyone in your household cared for an individual, who is in quarantine or is a presumptive positive or has tested positive for COVID-19?
🟨 Do you have any reason to believe you or anyone in your household has been exposed to or acquired COVID-19?
🟫 To the best of your knowledge have you been in close proximity to any individual who tested positive for COVID-19?
