By submitting this form, you agree to allow your son or daughter to swim at the Forest Park Pool unattended by yourself or any other adult. Only children 11 and over qualify for this status. You will not hold the Lifeguards, Pool Management company or the Forest Park Homeowners Recreation Association (FPHRA) liable for any injuries, disease or harm to your child.
