Included are forms for member registration with the GCFSC and
USFSA. This package includes membership applications, a Volunteer Survey,
Health Release/Waiver Form, Anti-Bullying Contract and a copy of the GCFSC Ice
Rules & Procedures. Please review the Ice Rules & Procedures with your
skater, sign, and submit it with the following items.

Fee of $50.00. If a check is used it must be dated June 30,2026.
GCFSC Forms
Complete the GCFSC Membership application, Volunteer Sign-Up Form,
Health Consent and Waiver of Responsibility and Hospital and Medical Release
Form, GCFSC Ice Procedures and Rules Form, Anti-Bullying Contract and
Professional Information (if applicable). New applicant applications will be
presented for approval at the next Board of Directors meeting.
Mid-Year Club Transfers
USFSA applications are to be processed through your “Home Club”.
If you are changing your home club, you must have a signature of release from
the previous home club on the USFS membership registration form before it can
be submitted to the membership chair of your new club. USFS Member Services will not process the change unless this form is
filled out in its entirety. By signing the “Previous Home Club Signature”
line, the membership chair of the previous club signifies that the departing
member has fulfilled all financial obligations. The new home club will not
accept the application if this line is not signed.
Restrictions
- GCFSC registrations forms must be complete to ensure timely
approval and competition eligibility. - Membership Application and fees must be
received before attending any Club functions after June 30,2025. This
includes summer activities, if any.
