Transportation Plan Form

Use this form to purchase an annual transportation plan for 2016-17 school year.

If you have more than one student riding the bus, please complete a form for each one.

    Bus Stops

    You can find the Bus Schedule, Fees and Rates, and Transportation Policy by clicking the links at the top and bottom of this page. Please select your primary AM and PM bus stops in the drop-down menus below. You may use either the same or different primary stops (for example, Gilroy Platinum Theatre in the AM and Morgan Hill Bible Church in the PM). If you need to have alternate stops for the AM and PM on some days, please select those also. If you do not need any alternate stops, please continue to Payment and Transportation Plans. Your fee will reflect the stop with the highest rate. PL = parking lot TS = transit stop All other stops are on the street in front of the business. ** Thistle Hut: If parent is not present when bus arrives, student will be dropped off at Castroville School.

    Payment and Transportation Plans

    You will be invoiced for transportation fees for the entire school year, but fees may be paid either annually by August 1, 2016 or monthly by FACTS. Monthly fees are deducted by FACTS based on your Tuition Option Plan. Please call the Business Office at 831.768.6112 with questions regarding FACTS.

    By submitting this form, you agree to the following policies:

    1. I have reviewed the 2016-17 MVCS Transportation Policy with my student and I acknowledge and agree that bus service is subject to my student’s compliance with that policy. 2. I will pay Monte Vista transportation fees corresponding to the transportation plan and payment options selected on this form. 3. I can change my transportation plan from two-way to one-way or vice-versa once at no charge. For any additional plan changes I will be charged a $50.00 processing fee. 4. I may cancel bus service at any time but will be held financially responsible for fees through the end of the month. I understand that cancellation must be made prior to the 25th day of of that month. I understand that a $50.00 processing fee will be assessed if I re-enroll for bus service after cancellation.
