Pickleball Sign Up
Each team will need to be made up of two people and will need to pay the $10 entry fee on K-12 Payment Center beginning September 2nd. No registrations will be accepted after October 20th. In the K-12 Payment Center, be sure to scroll down and choose STEM Pickleball Tournament to pay. EACH person needs to complete this form and sign the waiver to play.

Acknowledgement of Liability Waiver *

By checking the box below, I agree to participate in the Student and Staff Pickleball Tournament hosted by Northwest Cabarrus High School. I acknowledge that participation in this athletic event involves potential risks, including but not limited to physical injury.

I hereby agree to the following:

  1. I voluntarily assume full responsibility for any risk of injury or loss that may occur as a result of my participation in this event.

  2. I, on behalf of myself, my heirs, and assigns, release and hold harmless Northwest Cabarrus High School, Cabarrus County Schools, including their employees, staff, agents, and volunteers, from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during the course of the tournament.

  3. I certify that I am physically fit to participate in this activity and have not been advised otherwise by a qualified medical professional.

  4. I understand that all participants are responsible for their own safety, and I agree to follow all posted rules, staff directions, and proper conduct during the event.
