This form is part of our registration process and must be completed and submitted (one for each student) even if transportation is not being requested for the 24-25 school year. Additionally, if during the school year you need to make changes to transportation for your child(ren), please fill out and resubmit the form.  If computer access is limited please see the school building Principal.
