The self-nomination period for this application will be annually from October 1st through December 31st. Once applications are received via Google Form submission, they will be forwarded to your county/city 4-H office contact or active county/city All Star Chapter for review.
The information you provide on this form will be used by the Maryland 4-H All Stars to determine your eligibility for acceptance, one of the highest recognitions that you as a 4-H member may receive in Maryland. It is an honor as well as a responsibility to be asked to join the Maryland 4-H All Stars. It is an opportunity to continue your involvement with 4-H and provide service as a 4-H All Star.
Submission of this application does not guarantee this self-nomination form will be accepted as a successful application. All applications are subject to approval by the local county/city 4-H All Star chapter and/or the MD 4-H All Star membership committee as outlined in the MD 4-H All Star Constitution and Bylaws.
