Parent Information and Agreement
By filling out this form, I give permission for my child to attend the Cedar Park High School Project Graduation 2020 Party from May 28 to May 29 and participate in all activities. I also herby authorize any physician, nurse, trainer, sponsor, hospital, or other healthcare provider, as the volunteer or any of those listed deems necessary for the health and safety of my child. This authorization begins May 28th, 2020 for the duration of the Project Graduation sponsored event, and is made pursuant to Chapter 35 of the Texas Family Code. I further agree to release, hold harmless, and indemnify the CPHS Project Graduation sponsors from liability, damages, and/or claims, made by any person whomsoever, related to injury, treatment or care of my child.
Project Graduation is a substance free celebration honoring the 2020 graduating class of Cedar Park High School. I understand that if my child breaks stated rules, I will be called by one of the sponsors and I will be expected to pick up my child at Main Event.
My child will not be allowed to attend Cedar Park High School's Project Graduation Party unless this form is signed (electronic signature). I understand that this party is free to graduating seniors.
I understand that prizes will be given only to those graduates in attendance at the celebration event, and there is no guarantee that an individual will receive a prize.
Seniors must be chemical free. Any graduate that arrives under the influence will be taken to a designated area where parents/guardians will be notified to come pick them up.
I HAVE READ THE ABOVE RULES AND WILL BE ATTENDING THE 2020 PROJECT GRADUATION PARTY CHEMICAL FREE.
