1. Your gross income and tax deducted from all self-employed work.
2. Your expenses when you were self-employed.
3. Any P45’s or P60 from each employer you worked for under PAYE.
4. Any additional income details.
5. Your Unique Tax Reference number (UTR) from HMRC.
