If your child is a Registered SCN Member and is between the ages 0-6 Years, you may proceed to fill out the form. Your child's letter of registration must state that they are a registered member prior to November 1, 2025.

We cannot accept *pending* registration letters from SCN Membership department as we receive the registered members list directly from SCN Membership. The number of registered members on the list is the amount that we budget for, as stated in every 0-6 year gift card incentive.

BIRTHDATE between the dates of December 1, 2018 to November 30, 2025.

Our gift cards are NOT affiliated with any other SCN department
Only ONE parent or guardian may pick up the gift card; please arrange this accordingly.
We will only provide a gift card if ALL information is submitted correctly. Please note that sign-ups are NOT accepted over the phone for reporting purposes.
One form per child PLEASE put their name that is on birth certificate
