Thank you for your interest in participating in the 40th year of Beach Sweeps! Fall Sweeps is scheduled for Saturday, October 18, 2025.

**EACH INDIVIDUAL VOLUNTEER MUST COMPLETE THIS FORM TO PARTICIPATE IN THE BEACH SWEEPS.**

This form must be filled out and submitted for EACH individual participant.

Example: If a parent and their child would like to attend, this form must be submitted TWICE: once for the parent, and a second time for the child. A parent/legal guardian MUST complete this form on behalf of minors.

Waiver: By submitting this form, I hereby release and discharge COA and the Beach Sweeps sponsors, and their respective agents and employees from and against any and all liability, including for any losses, damages or injuries, including the contraction of a communicable disease, arising from my participation in the Beach Sweeps.  I grant full permission for COA to use photographs, videos, recordings or any other record from the Beach Sweeps, including those in which Participant appears.
*Please note: This form must be completed by an adult (age 18+). A parent/legal guardian must complete this form for minors. On the day of the Sweeps, children age 12 and under MUST be accompanied by an adult.
