OQA-2 Instructor Student Training Records Evaluation

This evaluation examines the quality of instructor's student records in PTR records, relative to the requirements of the LFS SOPs.

This evaluation form is completed for each Instructor specified by the Chief Flying Instructor in the audit request.

The sampling method is based on the 5/10/15 rule of selection. On the date and time that this evaluation is conducted, count back and identify the 5th, 10th, and 15th flight dual flight conducted by the evaluated Instructor. These form the samples to be evaluated. If the flight forming part of the sample is NOT adual training flight--if it is a rental check flight, for example--select instead the next preceding dual training flight. If you have questions contact the CFI.

IN THE EVENT that you answer "no" to any of the questions below, advise the CFI, and complete the LFS Non-Conformance or Unsatisfactory Finding Report, available on the school's website on the Education Quality Assurance page, linked from the Staff web page, or at the following link:

    Unsatisfactory Findings

    An unsatisfactory finding occurs when "NO" is answered to any of the above questions.
