Brilliant Minds Creative Kids Application Form
The NSW Government is funding the Creative Kids Program, which provides families of school-aged kids (4.5 to 18 years old) to get involved in creative activities. Parents, guardians and carers can claim a $100 voucher per child, per year with Brilliant Minds, a registered provider with Service NSW. Before completing this form, you must apply for your voucher through Service NSW ( - Usually takes no longer than a couple of minutes). Once you have successfully gained your voucher by Service NSW you may choose any one of THREE packages from the product list below.
