We need each family to fill out this form so that Westlake Charter has an accurate count of who has, and has not, filled out the Free and Reduced Price School Meals Application.

Each year, we ask families to complete a Free and Reduced Price Meals (FRPM) application. Even if you choose not to complete the FRPSM application, we still need this form completed for each student so that we can ensure every family had the opportunity to complete the application.
