Financial

Be advised that the tuition fee remains payable regardless of any holidays, long weekends, or Ramadan. A yearly registration and supply fee is applicable to all Institute of Knowledge Foundation (IOK) programs. The student and/or the guardian/parent of the student is responsible for purchasing the necessary resources, which are not part of the registration fee.

Upon registration, the registration and supply fee must accompany your child’s application form in order to reserve a place in the IOK program. Tuition, registration and supply fee are non-refundable under any circumstance. An NSF fee of $50 will be applied to all payments processed with insufficient funds. Late payments will be assessed a $50 surcharge for failure to pay by the date(s) specified. If a student is ineligible for admission before the semester start date, all fees for the semester paid will be refunded. Items not included in tuition fees: field trips, extracurricular activities, uniforms, transportation and events. You understand have read and agree to this program's terms, conditions and fees. You are aware that it is your responsibility to ensure sufficient funds are available in all accounts specified for payment. You authorize IOK to charge your account per the pre-authorized payments and timelines specified in accordance with your selections.

IOK requires a twelve (12) month commitment. If you, however, decide to remove yourself or your child(ren) from the program before the completion of the twelve (12) month commitment, then you must notify IOK in writing via email at programs@iokfoundation.org thirty (30) days prior to the withdrawal and agree to pay a cancellation fee of $250.00. If you decide to travel, you will still be charged the monthly fee depending on the length of the travel to hold the spot for you or your child in the program. Otherwise, you or your child’s spot will be given up to someone else. You aware of the cancellation policies and agree not to dispute or attempt to charge back the above signed-for and acknowledged charge(s).

Fees must be paid for the full month in which the student(s) begins school. Failure to maintain regular payments will result in revoking enrollment of the student.

Medical/Media/Photo Release

The medical information provided to IOK is to ensure that medical personnel have details of any health-related matters that may be required during treatment by emergency personnel i.e. ENT, first responder, and/or a physician.

IOK requires your consent that if a medical emergency involves you, that the emergency contact on file will be contacted. If the emergency contact on file cannot be reached, you authorize the calling of an emergency personnel i.e. ENT, first responder, and/or a physician to provide the necessary medical treatment in the event of an injury and/or if you become ill. In case of an emergency, you authorize to be treated by certified emergency personnel i.e. ENT, first responder, and/or a physician.

IOK requires your consent that if a medical emergency involves your child, you will be contacted. You hereby give IOK and its partners and affiliates consent to use and reproduce your child's name/image for promotional purposes related to IOK, its member programs, and/or external partners. Your child’s name (unless otherwise authorized) or image may be published or used in newspapers, promotional videos, television commercials, program brochures, posters, on the World Wide Web or otherwise displayed to the public or used for other educational/fundraising purposes, either in whole or in part by IOK, its member programs, and/or external partners. You release IOK and its agents from all claims, of any nature, based on any uses of the above. You, the student and/or parent/guardian of the child named in this form give permission for yourself and your child to participate in the programs and services of the IOK, and consent to any necessary first aid or emergency medical treatment is given or provided for yourself and/or your child, waive any claims against IOK, the sponsors of said programs, or any of the IOK representatives, employees or volunteers, in respect to any personal injury to yourself and/or your child or to any other person or any loss of or damage to property, arising in any way at, from or in connection with the programs and services of IOK. You are providing this waiver on behalf of yourself and/or your child and on behalf of your spouse and any other family members or other persons who might be entitled to assert such a claim.

Enrollment for your Child

Once all admission procedures have been completed, you will be notified of the decision regarding your child. If your child is accepted, you will be required to enter into a contract with Institute of Knowledge Foundation (IOK), acknowledging the role that you are required to pay the outlined amounts.

Liability Release/Waiver

You understand that although the program and/or class will be staffed by IOK personnel who will do their best to ensure the well-being of your child, you shall not find IOK and/or their staff members, employees, and volunteers liable for any injury or loss of property. Furthermore, you understand that it is your responsibility to instruct your child to observe proper Islamic behaviour and obey the regulations set by IOK staff members, employees, and volunteers.

IOK and/or their staff members, employees, and volunteers are not liable for any property damage or personal injury which may arise during the transportation of your child to and from school by you/your spouse or a third party. The undersigned releases every person and institution from all liability resulting from or pertaining to the furnishing of records, documents, and other information provided to IOK for that purpose.

Trips/Outdoor and/or Offsite Visits

You hereby grant permission for your child to participate in trips, outdoor and/or offsite visits, which are organized by IOK.

You acknowledge that these trips, outdoor and/or offsite visits are organized and supervised by IOK and/or their staff members, employees, and volunteers.

You understand that reasonable precautions will be taken to ensure the safety and well-being of your child(ren) during these trips, outdoor and/or offsite visits.

You authorize IOK and/or their staff members, employees, and volunteers, to seek necessary medical treatment in case of an emergency or accident during these trips, outdoor and/or offsite visits.

You understand that it is your responsibility to ensure that your child arrive at IOK premises on-time and he/she is collected promptly at the end of the these trips, outdoor and/or offsite visits.

You will inform IOK and/or their staff members, employees, and volunteers, in advance if someone other than yourself will be picking up my child.

You agree to provide appropriate clothing, footwear, and any other necessary items for your child(ren)’s comfort during these trips, outdoor and/or offsite visits.

You acknowledge that your child will be expected to follow the instructions and guidelines provided by IOK and/or their staff members, employees, and volunteers during these trips, outdoor and/or offsite visits.

You release IOK and/or their staff members, employees, and volunteers from any liability or claims arising out of your child(ren)’s participation in these trips, outdoor and/or offsite visits.

You understand that photographs and/or videos may be taken during these trips, outdoor and/or offsite visits, and you grant permission to IOK and/or their staff members for your child’s image to be used for educational or promotional purposes related to IOK programs.

You certify that you have read and understood the terms and conditions outlined above, and you consent to your child’s participation in these trips, outdoor and/or offsite visits.

I have read and agree to the rules, policies and procedures, including the most recent school code of conduct (found on the school website). I am aware of the disciplinary policies and agree not to dispute or attempt to dispute or hold liable the Institute of Knowledge Foundation (IOK), their agents, their staff or their directors due to decisions made concerning my child(ren)’s admission or enrollment at IOK Islamic School.
