I hereby choose to participate in the Pride of the Northside 4 on 4 tournament (including activities, i.e. Dunk contest, 3 point contest, etc.) and I am responsible for myself (and my teammates are responsible for themselves) in the event of injury or health related problems stemming from my involvement in this event, and completion and submission of this form acknowledges that I am in full agreement with all rules and regulations of the tournament. Pride of the Northside is not liable for injuries, health problems etc.

I agree to respect all players, spectators, referees, and staff of this event regardless of any circumstances. I will respect the community of North Edmonton, and failure to do so will result in my automatic ejection from games and potential ejection from the event with absolutely no chance of refund or reimbursement--no exceptions. Negative behavior will also determine my (and my team's) future involvement in the Pride of The Northside Tournaments.
