Orders shipped to, or billed to, countries on the U.S. Department of Commerce Country Group D, will require an Export End Use Certification form and are prevented from booking until the Export End Use Certification form is attached. The completed, signed form must accompany the contract/purchase order at the time of submission.

Please choose the type of form you want to submit. The data filled in the individual form applies only to the current request (PO/Order). The yearly form on the other hand will be valid for all the requests for one year, from the date of submission.
