The purpose of this form is to request the use of the sound and projection system at The Potter’s House Family Worship Center (PHFWC). Every effort will be made to accommodate your event, but the request may not be confirmed if there are scheduled events that conflict or if this request form is not turned in at least 3 weeks in advance of the event date.
The Potter’s House Family Worship Center reserves the right to deny any sound/projection request. If you have any questions or concerns, please contact the church office at 989-772-5681 or email office@pottershousefwc.org

Request for sound and media use requires a PHFWC sound technician. All PHFWC sound technicians must be confirmed through the sound/visual ministry leader.
