Booking Request Form
Thank you for your interest in working with me! Currently I am not accepting new clients.  After you fill out the booking form we will still schedule a consultation and if we are good fit I will add you to my waitlist.

Please remember that I do not work with clients under the age of 18 and that I do not accept insurance

Please set aside 5-10 minutes to complete this form. I will review your response before our consultation to ensure we make the most of our time or so I can get you a referral.

Note: I can only provide therapy services to Texas residents. Coaching services are available anywhere in the US.

I will reach out within 24-48 hours with next steps and/or to confirm scheduling, so be on the lookout! Be sure to check your spam folder if you don't hear back from me.

Are you interested in telehealth or in-person services?  *
On a scale of 1-3 how ready are you to invest in your healing today?  *
I'm not ready yet, I'm just curious about your work and what you have to offer.
I'm ready to invest. I've thoroughly reviewed your website, the services you provide, your about me, and your investment fees. I feel like we're gunna vibe. I'm at the point where I'm ready for something to change in my life and take it to the next level. Let's do this!
What kind of financial investment are you ready and available to make in your healing today?  *
It's important to note that this question is in no way a contract. Two of my core values are transparency and authenticity. In service to this, I like to be up front about the financial commitment that working with me entails. Every treatment plan and investment looks different for everyone but I feel it's important to be direct about pricing of services so you can make the best decision for yourself.
What, if anything, have you tried previous to therapy to feel better?  *
Have you ever participated in therapy before? If yes, how was your experience? What did you enjoy and what do you wish was different? If you haven't tried therapy before, what has held you back?  *
How long do you expect to be in therapy?  *
How much time are you willing to invest in therapeutic work such as journaling, reading books, and practicing new skills outside of sessions?  *
Please briefly share what issues are bringing you into therapy at this time: *
What concerns do you have about therapy (if any)? *
How severely are your current symptoms impacting your day-to-day life?  *
How long have you been considering therapy?  *
Vibe in the therapeutic relationship is important. Why do you want to work with me?  *
What questions do you have for me about myself or my practice?  *
You may end up with more questions on our call. This question is just here to get you thinking about what you may want to ask me ahead of time. If you need an idea of a few example questions, here's a blog that may help!
How did you find my practice?  *
Required
I reserve Tuesday 12-1pm and Friday 3-4pm for consultation calls. What is your preferred day and time to schedule within this time frame?  *
Please note that calls typically last 10-15 minutes and you will want to be in an environment that is quiet with good reception. Additionally, if this time block does not work for you, please share what times/days do work and we will find a mutual time that works for us.
Is there anything else you would like me to know before we jump on a call?
