PAL CAMP SIGN-UP FORM
Thank you for showing interest in participating for AYSC's PAL Camp!

PAL Camp is an annual camping trip that all AYSC members have the opportunity to attend and enjoy activities! Bonding activities range from water balloon fights, obstacle courses, and cabin decorating, to night hikes and eating s'mores and other good food around the campfire.

In order to attend, please digitally fill out ALL FOUR WAIVERS and email them to miaomiaoli0320@gmail.com. Participants will be required to attend ALL committee meetings, show part in their respective committees, have waiver and forms signed, and understand/agree to follow all rules/guidelines listed!  

Location: Oakland PAL Camp (10100 Skyline Blvd, Oakland, CA 94611)
Rides will be provided, but members must be dropped off at CCUMC (321 8th St, Oakland, CA 94607).
Duration: August 4th - August 7th 

All forms must be completed by July 20th (If you have questions about the process of filling out the form, feel free to dm us @youthsofaysc).

Rides are provided to the campsite, but you must come to CCUMC (time will be mentioned eventually).

  • You have notified AYSC at least three days prior to the event through this form.
  • If the form is submitted two days prior, there is a possibility a ride cannot be provided.
By agreeing to this, I have sent the signed waivers to miaomiaoli0320@gmail.com and agree to the conditions listed here OR I will sign the physical waiver and will turn it in before Pal Camp.  *
