A few things to know for this form:
1. You and at least one parent/guardian need to go through this form TOGETHER!
2. The form will take you 25-35 minutes to complete
3. Have an open mind!  The fun of band is less what instrument you play and more about the music and experiences of being in band with your friends!  
4. At the end of this form you will indicate which instruments you think might be a best fit for you.
