Complete the electronic form using the event summary checklist and worksheets made available to you by your Coalition contact.
- Submit all forms and backing documents within 30 days of the cleanup and no later than June 1, whichever comes first.* - Weights must be filled in and have accompanying transfer station slips for reimbursement. - Be as exact as possible (do not round if you know exact quantities). - Please present quantities in units requested (by tons, count, etc). Tonnage is listed on weight slips. To convert pounds to tons, divide the pounds by 2,000.- Submit backing documents which include weight slips, receipts, invoices, and samples of promotional materials to your Neighborhood Coalition office.
