By signing below, I acknowledge and agree to the following:

1. Voluntary Participation: I am voluntarily participating in the Contact Improv Jam with full knowledge that this activity involves physical movement and potential risks of injury.

2. Assumption of Risk: I understand the nature of Contact Improvisation and assume full responsibility for any injuries or damages that may occur during my participation, whether caused by my actions or the actions of others.

3. Responsibility for Self & Others: I agree to move with awareness and care, to honor my boundaries and those of others, and to communicate clearly when needed.

4. Consent & Touch: I understand that this is a consensual, contact-based practice, and I am free to say “no” or set boundaries at any time. I will also respect the boundaries and signals of others.

5. Release of Claims: I release and hold harmless the organizers, facilitators, venue owners, and fellow participants from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage I may experience during the event.

6. Health Statement: I affirm that I am in good physical condition to participate, and will refrain if I am feeling unwell or am recovering from an illness or injury.
