Please only use this Form to submit a Health Report or Cause Of Death if you are an Irish Water Spaniel breeder or owner.

You can use this Form to enter a Health Report for ONE Irish Water Spaniel (born any time from 1995). Please submit a new Form for each additional Irish Water Spaniel.

By using this Form you agree to the Irish Water Spaniel Health Council using the data you supply to inform their Health Plans and to provide reports on the health status of Irish Water Spaniels.

Your personal details and those of your Irish Water Spaniel(s) will not be shared publicly and will only be used, in confidence, by the Breed Health and Welfare Sub-Committee.
