2026-27 South Mountain YMCA School Age Child Care Schedule Update Form
Please use this form below to make a change to your child's School Age Child Care Schedule or cancel enrollment. Changes will only be approved if there is availability. Our cancellation policy is 2 week notice.

- ALL REQUESTS RECEIVED BETWEEN AUGUST 1st- SEPTEMBER 14TH WILL NOT BE PROCESSED UNTIL THE WEEK OF SEPTEMBER 21ST.
- Any requests made before the DEADLINE of 12pm on Monday will be implemented for the FOLLOWING Monday of the upcoming week
- However, any changes requested AFTER Monday 12pm will be effective starting from the SUBSEQUENT Monday of the week after

Section 3 : Change to Schedule (There is a $35 change fee per request)

Section 4: Cancellation Request (2 weeks notice required)
