Information collected with this form is used to document trail work done, plan future maintenance work, and the volunteer hours are reported to the federal entity responsible for each section (USFS, NPS, etc.).

For each maintenance outing on the OHT, please submit a maintenance details form (single form per outing) and volunteer demographics forms (one form for each volunteer).
