If you live in the Auburn School District, and would like to request that your elementary student be permitted to attend in a school other than your home school, please complete this form. Submission of this form does not guarantee permission, as student enrollment is based on availability and all decisions will be made accordingly. Parent/Guardian is responsible for timely transportation. If unexcused absenteeism or tardiness should become an issue, your child's placement may be reconsidered. Students are expected to meet school expectations for behavior and academic work. If you desire this change again next year, you will need to submit a written request at the end of the school year.
