Tucker High School Transcript Request Form
Please select the following items that you need submitted to the college(s) of your choice. The Counseling Department will process each request submitted. Payment must be received before request is processed. This form must be completed before submitting the processing fee to the Counseling Office. This form is to be completed by those persons who graduated from Tucker High School from 1997 and beyond. If you graduated prior to 1996 please go to http://www.dekalbschoolsga.org/how-to-obtain-student-records-transcripts/records-request-form/ to request records and transcripts. This form is only to be completed if Tucker High School was the last school you attended or graduated.
