COVID-19 and the UConn Nation - Documentation Submission Form
Archives & Special Collections (ASC) is documenting the wide range of recent reactions, experiences, and activities undertaken by members of the UConn Nation as we all adjust, struggle and move forward through the challenges of a world-wide pandemic.

We're reaching out to the UConn community–students, faculty, staff, administrators, alumni, and other UConn community members–to share your stories, in whatever form you wish, to be collected, preserved for posterity, and made accessible for research and study in the UConn COVID-19 Collection. Examples can include audio files, social media posts, emails, screenshots, photographs, blog posts, journaling, zines, interviews, and more.

If you are interested in donating your documents, photographs, recordings, or other personal materials to the UConn COVID-19 Collection, please complete this form. After we receive your information, we’ll send you a link to a Google Drive folder to upload your files. The folder will contain a deed of gift for you to sign. This form documents your contribution and indicates how your donation may be used. If you have physical material you'd like to donate, we will follow up via email to make arrangements for transferring this material to ASC.

Materials collected as part of this documentation will be reviewed and processed before access will be granted; given the unusual circumstances resulting from the COVID-19 pandemic, this may take some time before the collection is available for research.

I understand that the purpose of this submission is to collect personal experiences of individuals affiliated with the University of Connecitucut related to the COVID-19 pandemic for inclusion in the permanent collections of the University of Connecticut. The submitted materials serve as a personal record and may be used as a scholarly and educational resource for the University of Connecticut and the general public.I understand that the University of Connecticut plans to retain the product of my participation in the COVID-19 documentation project, including but not limited to my interview, presentation, video, photographs, statements, name, images or likeness, voice, and written materials (“My Collection”) as part of its permanent collections.I hereby grant to the University of Connecticut a non-exclusive, irrevocable, perpetual, worldwide, royalty-free license to use, reproduce, transmit, display, perform, prepare derivative works from, distribute, and authorize the redistribution of the materials in My Collection in any medium. By giving this permission, I understand that I retain any copyright and related rights that I may hold.I hereby release the University of Connecticut, and its assignees and designees, from any and all claims and demands arising out of or in connection with the use of My Collection, including but not limited to any claims for copyright infringement, defamation, invasion of privacy, or right of publicity. Should any part of My Collection be found to include materials that the University of Connecticut deems inappropriate for retention, the University of Connecticut may dispose of such materials in accordance with its procedures for disposition of materials not needed for the University of Connecticut collections. *
