I understand that as part of the application process for employment with Nor Ala Day Camp, the camp may conduct a pre-employment background check, including but not limited to:

Criminal history records
Child abuse and neglect registries
Sex offender registries
Verification of past employment and references

I hereby authorize Nor Ala Day Camp and its representatives to conduct such background checks and investigations as necessary. I release the camp and any individuals or organizations providing information from any liability in connection with the background check.

I certify that all information provided in this application is true and complete to the best of my knowledge. I understand that providing false or misleading information may result in denial or termination of employment.
