Acknowledgment of Risk:
I, the undersigned, understand that participation in soccer activities, including but not limited to practices, games, training sessions, and events organized/hosted by the Barrie Cardinals Soccer Club ("the Club") or ("The Nest), involves inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, and other serious injuries.

Release of Liability:
In consideration of being allowed to participate in Club activities, I hereby release, waive, and discharge the Nest and The Barrie Cardinals Soccer Club, its directors, officers, coaches, volunteers, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, including death, that may be sustained by the participant, or to any property belonging to the participant, while participating in Club activities.

Medical Treatment Authorization:
In the event of an emergency, I authorize the Club representatives to secure necessary medical treatment for the participant. I understand that I am responsible for any medical expenses incurred.

Assumption of Full Responsibility:
I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury that may be sustained by the participant, or any loss or damage to property owned by the participant, as a result of participating in Club activities.

Insurance Acknowledgment:
I acknowledge that the Barrie Cardinals Soccer Club or "The Nest" does not provide medical or accident insurance for participants. It is my responsibility to provide adequate insurance coverage.

Agreement:
I have read and fully understand this Injury Waiver and Release of Liability. I acknowledge that I am signing this document voluntarily and that it shall be binding upon me, my heirs, executors, administrators, and assigns.

1. Acknowledgement of Risk

I understand that participation in physical activities at The Nest Summer Camps hosted by The Barrie Cardinals soccer Clun de Futbol, involves inherent risks, including but not limited to the risk of concussion or other head injuries. I acknowledge that concussions can have serious and lasting effects on physical and cognitive health.

2. Concussion Education

I confirm that I have reviewed and understand the signs and symptoms of concussions, which may include but are not limited to:

• Headache or "pressure" in the head

• Nausea or vomiting

• Dizziness or balance problems

• Blurred vision

• Sensitivity to light or noise

• Difficulty concentrating or remembering

I agree to report any signs or symptoms of a concussion immediately to the camp staff and to seek medical attention as needed.

3. Return-to-Play Policy

In the event of a suspected concussion:

• The participant will be removed from physical activities immediately.

• The participant may not return to activities until cleared by a licensed medical professional.

• A written medical clearance will be required before resuming participation.

4. Waiver of Liability

I, the undersigned, understand and accept that The Nest Summer Camps and Barrie Cardinals Soccer Club are not responsible for any concussion or injury that may occur during camp activities. I waive, release, and discharge The Nest, Barrie Cardinals Soccer Club de Futbol , its staff, volunteers, and affiliates from any liability related to concussions or other injuries sustained during camp participation.

5. Parental/Guardian Consent (if participant is under 18)

I, as the parent/guardian, acknowledge that I have read and understood the information provided above. I give permission for my child to participate in The Nest Summer Camps and agree to the terms of this concussion waiver.

1. Payment Terms:

   • Payment must be made in full upon receipt of invoice and prior to the participant's first day of camp. No exceptions. 

   • We accept payments  cash, or through Quickbooks "pay and review"  with credit card , and debit cards 

   • via e-transfer to cardinalevents@barriecardinals.com 

2. Campers Refund Policy:

   • All registration fees include a non-refundable deposit of $50.

   • Refunds for withdrawals made 14 days prior to the camp start date will be issued minus the non-refundable deposit.

   • No refunds will be provided for withdrawals made within 14 days of the camp start date or after the camp begins, except in cases of documented medical emergencies.

   • Refund requests must be submitted in writing to The Nest Summer Camps administration at cardinalevents@barriecaridnals.com 

3. Missed Days:

   • Missed camp days due to illness, injury, or personal scheduling conflicts are non-refundable.
