Distance Learning Submission
By submitting this form you are enrolling your child/children in the OCS Distance Learning program for at least the first quarter. Students may return to the in-person learning format at the end of the quarter. If requested in this form, OCS will prepare devices and take home items to for each student submitted. Students devices will be available for pickup the week before school begins. In addition to the sign-up form, every student must have an educational development plan and distance learning privacy form on file filled out with the school prior to the start of the school year. These forms can be filled out when picking up student devices. OCS will be in touch with additional information. Thank you for your submission.
