This form is to request a refund. Please remember the National Membership Fee, Credit Card Fee and Late Fee is nonrefundable that was also on registration form. Refund comes in check form that could take up to six weeks before the next check signing day.  Once a player has participated in practices or games, a refund will not be issued.
