MiHIA Community Health Excellence Awards
MiHIA’s Health Scorecard allows your organization to consider evidence-based health improvement strategies to identify and consider programming, interventions or policies that could be implemented within your organization, helping all stakeholders shift to a focus of well-being.
How it works:

Once your Health ScoreCard form and supporting documentation have been submitted, your Community Health Excellence Award submission is complete.
Within 5-days of submission, MiHIA will confirm if your submission has been received. The MiHIA Evaluation Team will review and calculate scores following the 2019 submission deadline; additional information may be requested at that time. MiHIA will also offer guidance and support as you and your organization set an organizational 3-year improvement goal and develop an action strategy.
