The purpose of this form is for our families to submit a COVID Positive Test or Close Contact to our office. All families are required to notify our office immediately of a confirmed positive test, possible close contact or COVID symptoms, so we can take appropriate next steps to continue to uphold our commitment to safety and health.

Due to confidentiality, this form is ONLY to be used to report any cases within your family,  not to notify us on behalf of another family.
