The Canadian Scientific and Christian Affiliation (CSCA) would like to use student's names in publications about student scholarship winners. For the entirety of this form, "publications" includes but is not limited to the CSCA Newsletter, the CSCA website (csca.ca), and their social media accounts, such as Facebook and Twitter.
(This is required, but you may opt to leave out your last name.)
The CSCA will also use your school's name and location in publications about student scholarship winners.

If you win, it will be required for you to submit a reflection on the conference (due 2 weeks after the chosen conference ends; see main announcement for details), together with at least one photo showing that you were there, before any scholarship funds will be dispensed. The CSCA may use either or both of these in its publications.
