As per PA state law there are required documents/information needed in order to be be successfully enrolled in school. This form will guide you through that process

Parental Consent/Health Record

This form MUST be completed at the beginning of EVERY school year.

It allows you to add any allergy or health information that can aid in the care of your child, and you can give consent for nursing to administer Over-the-Counter Medication as per our standing orders from our school doctor.

It also alerts the nursing office to any new allergy or health condition your child may have.
