2019 Summer Camp Change & Cancellation Form
A cancellation form is required for all schedule changes and cancellation requests. No refunds are granted for cancellations made with less than ten business days' notification prior to the start of a course. All changes and cancellations are subject to a $20 fee per camp. (Black Rocket program cancellations carry a $50 fee/student/camp.) Please complete one change/cancellation form per student.
